Recipe - Compounding Worksheet

Verapamil Oral Suspension (LOW STRENGTH)   8 mg/mL
Lot No Expiry(mm/yyyy) Ingredients Master Formula
    verapamil  80 mg  TAB   
    Water, sterile        
    Simple syrup      qs to 
Recipe Total Quantity:
Final Quantity Desired: mL

Directions: 1. In a mortar, crush tablets and triturate to a fine powder. Levigate with water to form a paste.
2. Gradually levigate paste with simple syrup to form a liquid.
3. Pour into a graduated cylinder.
4. Rinse mortar and qs to final volume with simple syrup.
5. Transfer to final container and label.
