Recipe Total Quantity:
Final Quantity Desired: mL

Directions: 1. Withdraw required amount of midazolam from vials using a syringe and needle.
2. Transfer into a graduated cylinder and qs to final volume with simple syrup.
3. Transfer to final container and label.

Notes Midazolam injection may contain benzyl alcohol.

Can substitute midazolam USP powder for injectable midazolam.

Arbitrary expiration date of 14 days refrigerated can be assigned based on USP 795 guidelines.
