Recipe - Compounding Worksheet

metroNIDAZOLE in Chocolate Simple Syrup    15 mg/mL *Note Strength*
Lot No Expiry(mm/yyyy) Ingredients Master Formula
    metronidazole  250 mg  TAB   
    Water, sterile       to wet tablets 
    Chocolate - Simple syrup (SickKids Formulation)       qs to  
Recipe Total Quantity:
Final Quantity Desired: mL

Directions: 1. Place tablets and water in mortar. Allow mixture to sit for a few minutes until tablets soften.
2. Levigate mixture into a paste.
3. Gradually levigate chocolate-simple syrup with paste until a liquid is formed.
4. Pour into a graduated cylinder.
5. Rinse mortar and qs to final volume with vehicle.
6. Transfer to final container and label.
