Recipe - Compounding Worksheet

Vancomycin Oral Solution (prepared with 1 gram vials)    50 mg/mL *Note Strength*
Lot No Expiry(mm/yyyy) Ingredients Master Formula
    vancomycin  1 g  INJ   
    Water, sterile        
Recipe Total Quantity:
Final Quantity Desired: mL

Directions: 1. Reconstitute each vancomycin vial with 20 mL of sterile water and mix well.
2. Transfer to a glass amber bottle and label.

Notes Dose can be mixed with flavouring (syrups) or an acidic (apple or orange) juice just prior to administration to increase palatability.
Use mixture immediately. DO NOT store mixture once the flavouring agent has been added.

Storage in amber glass bottle is preferred, but can also be stored in amber plastic bottle.
