Ketamine Oral Solution (prepared from powder)    10 mg/mL
Lot No Expiry(mm/yyyy) Ingredients Master Formula
    ketamine USP     PWD   
    Water, sterile        
    Cherry syrup      qs to 
Recipe Total Quantity:
Final Quantity Desired: mL

Directions: 1. In a mortar, triturate ketamine powder to a fine powder and dissolve in water.
2. Pour into a graduated cylinder.
3. Rinse mortar and qs to final volume with cherry syrup.
4. Mix well, transfer to final container and label.

Notes This recipe should be compounded instead of the recipe prepared from injection whenever there is a limited supply/shortage of ketamine injectable.
