Vancomycin Oral Solution (prepared with 10 gram vials)   25 mg/mL (LOW STRENGTH)

    vancomycin  10 g  INJ   
    Ora-Sweet® vehicle       
    Water, sterile        
    Water, sterile       Use to reconstitute vancomycin vials - see notes section  
Recipe Total Quantity:
Final Quantity Desired: mL

Directions: 1. Reconsitute each vancomycin vial with 95 mL of sterile water (see Notes section).
2. Mix Ora-Sweet® and sterile water together (1:1).
3. Withdraw reconstituted vancomycin from vials and dilute with Ora-Sweet®/ sterile water (1:1) mixture.
4. Transfer to final container and label.

Notes The final volume of one 10 gram vial after reconstitution with 95 mL is approximately 100 mL.

Refrigerated storage is preferred.
