Pyridoxine (Vitamin B6) oral suspension   25 mg/mL *NOTE STRENGTH*

    pyridoxine  100 mg/mL  INJ   
    Oral Mix™    PO SOLN  qs to 
Recipe Total Quantity:
Final Quantity Desired: mL

Directions: 1. Withdraw required amount of pyridoxine from vial.
2. Transfer to graduated cylinder.
3. QS to final volume with Oral Mix. Mix well.
4. Transfer to final container and label.

Notes Pyridoxine (Mylan) 100 mg/mL injection contains benzyl alcohol 1.5%.
May substitute Oral Mix with Oral Mix SF.

Packaging:  glass or NON-PVC plastic amber bottles or oral syringes
